The pilot implementation shows end-to-end exchange from a proprietary EHR-structure to an open standards FHIR-storage, and view of data in a simulator-application and use for analytics. Users of the terminology in Iceland will also have access to new resources as they are developed. The eHealth policy in Iceland includes the implementation of an integrated EHR at a national level, which is securely accessible to authorized professionals at point of care. Furthermore, it allows people in the general public access to their own health data. This is achieved by integration of regional databases and the development of a national patient portal. The application, combined with information from the national prescription database, provides physicians with a much better overview of healthcare provided to their patients. Hence, this supports increased patient safety and quality of healthcare. Current plans to improve clinical documentation and national reporting include the implementation of a standardized terminology for X- ray diagnostics, documentation of symptoms, and physical examination. The product of a national collaboration between the responsible partners for classifications and terminologies, these plans are in coherence with future EHR development plans at a national level. A nationwide surveillance system on communicable diseases for the office of the Chief Epidemiologist of Iceland has been in use for some years. Recently, the system was updated to allow for real-time reporting on communicable diseases. SNOMED CT terminology is utilized in the system to ensure correct registration of pathogens. International Diet Dysphagia Standardisation Initiative Back International Diet Dysphagia Standardisation Initiative Clinical Partner In 2022, SNOMED International and the International Diet Dysphagia Standardisation Initiative (IDDSI) signed an agreement to incorporate IDDSI Framework v2.0 content into SNOMED CT to enable use in electronic health records globally. The agreement has been driven by community requests as well as by the need identified by the SNOMED International Nutrition Care Process Terminology (NCPT) Clinical Project Group to include IDDSI Framework v2.0 content in order to facilitate the inclusion of NCPT in SNOMED CT. Additional information Content has been created for each framework level, consistent with the IDDSI standard terminology and definitions subject to SNOMED CT editorial guidance, while additional content is being added to facilitate recording the use of the framework and diet scale together with related findings and interventions. As of January 2023, the content is included in the SNOMED CT International Edition. Clinical Decision Support (CDS) systems support healthcare providers to make well informed decisions at relevant points in the patient journey, such as diagnosis, treatment and follow-up. SNOMED CT enables these CDS systems to select the most relevant clinical guidelines, knowledge and alerts for an individual patient, by leveraging its comprehensive clinical hierarchies and defining relationships. The Decision Support with SNOMED CT guide introduces the key components of CDS systems, and explores ways in which SNOMED CT can be used to enhance the capabilities within each of these components. International developments promote the exchange and secondary use of health information, which requires interoperability of health data. SNLs are potentially interoperable due to their standardized and coded format and can be integrated into electronic health records (EHRs). New Dutch legislation and the European Health Data Space Act mandate the use of SNOMED CT for information exchange. Therefor the Dutch Nursing Association (V&VN) and the Dutch National institute for IT in Healthcare (Nictiz) developed a Dutch Nursing SNOMED CT Subset (DNSS) of patient problems. However, SNOMED CT and SNLs have different objectives: SNLs aim to support clinical reasoning and documentation of nursing care, while SNOMED CT aims to provide a global medical vocabulary to enable communication between EHR systems and analysis of information. To enable nurses use of their preferred SNL for primary documentation in the EHR, and exchange information with SNOMED CT a mapping is required between SNLs and SNOMED-CT. This mapping was executed by a working group with OS and NANDA-I experts, V&VN representatives and Nictiz terminology experts. The SNOMED CT International Edition to be published on first day of each month BLOG: The SNOMED CT International Edition to be published on first day of each month The SNOMED CT International Edition to be published on first day of each month Back 19 May 2023 Back In January 2022, SNOMED International moved to releasing the International Edition of SNOMED CT at the end of every month , rather than just twice a year. Today, we are announcing the move to a new release day: the first day of each month. In this blog, SNOMED International Release Manager Andrew Atkinson explains why we made the shift, how it reflects our ongoing commitment to innovation and evolution throughout the entire lifecycle of a release, and how it benefits our users. Q: What change will users see as of September 2023? AA: The International Edition packages will be released on MLDS on the first of each month, instead of the last day of the month. The final release on the old schedule will be on July 31, 2023, and the first release on the new schedule will be published on September 1, 2023. This means there will be no release in the month of August 2023, as there would be no new content to publish on August 1 after just having deployed the July release on the day before! This will be the only month to go without a release (including August 2024), as after that there will be a new release published indefinitely on the first of each month. Q: Why did SNOMED International and the Community decide to change the release day to the first of the month? AA: There were multiple drivers for this change, from removing the ambiguity of release dates to preventing conflicts with certain types of extension content. However, the predominant reason is to improve the efficiency of the release cycles, in order to provide benefits such as additional time for those stakeholders who base their extension and derivatives on the International Edition releases. Q: What problems will the new release day solve? AA: As mentioned, it removes any confusion as to which date releases will be published (previously they could be on the 28th, 29th, 30th, or 31st). Q: What kind of stakeholder engagement did we conduct to get feedback on this change? AA: We have discussed this proposal at great length with a variety of stakeholders, across multiple conferences. We also published early visibility notifications in the recent April 2023 International Edition announcements and Release Notes, and put a notice in our Early Visibility page - both providing the means for users to share their comments with us: https://confluence.ihtsdotools.org/display/RMT/2023+Early+Visibility+Release+Notifications Q: What impact does this have on national extensions or derivative releases? AA: This should have very little impact on extensions and derivatives, other than a positive one, which is to provide a bit of extra time for translations and migrations that are based on the “January” and “July” releases. That’s because these dependent releases will be brought forward from January 31 and July 31, to January 1 and July 1 respectively (instead of being moved back to February 1/August 1). This will prevent any confusion as to which releases are termed “January”/”July,” and will provide additional time for stakeholders to prepare their extension/derivative content that is dependent upon these releases. Q: What does this mean for the next monthly release? AA: The May 2023 International Edition release will be published as normal on May 31, as will the June and July releases (on June 30 and July 31 respectively). The first release on the first of the month will be published on September 1, 2023, and we will then continue to publish all subsequent releases on the first of each month. Concerns or questions? Finland’s commitment to SNOMED CT sets a foundation for clinical data to be exchanged accurately and consistently across Finnish health and social care settings, further extending the reach of structured clinical terminology throughout the European region. To make best use of SNOMED CT with available resources, Finland has chosen not to translate the whole terminology. Instead, the aim is to focus on two national reference sets that meet local needs in areas where there is international experience and models showing implementation success. The first target is an obvious one: standardizing the terminology and codes for anatomic pathology in the clinical laboratories. Finnish pathology laboratories have used SNOMED 2 since 1980’s. But lack of coordination eventually resulted in several dialects and variance between hospitals. Now the ongoing project aims at national reference set to help the biobanks, regional and national quality registers and structured reporting on individual patient basis. The other chosen application for SNOMED CT is patient problem list. Finland decided to use CORE Problem List as foundation for the local effort. CORE Problem List originated from a respectable group of medical institutions in the U.S. and is maintained by the National Medical Library thus making the acceptance of the reference set by clinicians so much easier. The ongoing project translates terminology into Finnish and when finished will cover terms for both professionals and patients. Finland's aim is to have these first two applications ready at the first half of 2020. SNOMED CT National Release Center will gradually take off during 2019 and 2020 to meet the needs for services and support during SNOMED CT local implementations. Back 18 Sept 2025 Back SNOMED International has introduced a new suite of digital tools that support internal collaboration, external knowledge sharing, and the broad global community of practice. These changes align with the organization’s goal of improving usability, performance, and access. The following solutions are now live and available for use: SNOMED Documents – The official home for SNOMED CT documentation and guides. The SNOMED CT Document Library is a centralized official repository maintained by SNOMED International and includes specifications, implementation, practical and user guides. SNOMED Forums – The community discussion platform for SNOMED International providing discussion categories, announcements, feedback, group collaboration, and user support. We encourage all members of the SNOMED International community—whether you are a clinician, implementer, researcher, or policy leader—to explore these new platforms and take part in shaping their growth.
