A good and precise translation of SNOMED CT is essential to improve the clinical documentation with a clear terminology, understandable for all healthcare professionals involved in the clinical data capture.
In our hospital, better clinical documentation offers many direct benefits for patients and clinicians, including data sharing and reusing, problem-oriented charting, dashboard, clinical decision support for better care and safety of the patients, and effective retrieval of patient data for research and statistics.
Scope
Belgium is member of SNOMED International since 2013, and has designated SNOMED CT as the recommended clinical reference terminology for clinical information systems. Implementing SNOMED CT in electronic health records ensures consistent and structured clinical documentation across healthcare providers and systems. This enables international interoperability, improves patient safety and allows effective use of data for research, analytics, and public health.
How SNOMED CT will be used
In our academic hospital, SNOMED CT is already implemented in different parts of the electronic health records for documentation of diagnoses (problem list), medical history, family history, surgical history and non-drug allergies. We also would like to extend its use to other domains such as pharmacy and laboratory. Translation is a key element to improve the compliance of our health providers with structured data capture.
