University of Nebraska Medical Center pathology cancer reports have been recorded and stored as Portable Document Format (PDF) in the Electronic Health Records (EHR). Therefore, the data held within these reports is not readily available for use in research or patient care decision making. To rectify it, we developed an approach to extract and SNOMED CT encode large amounts of historical cancer pathology data and insert these data into a Clinical Data Warehouse (CDW) to support clinical research endeavors and population health efforts. A five-step process was applied to five cancer types: ampulla of Vater, pancreas, esophagus, lung, and prostate.1) PDF pathology reports were generated from a pathology information system; 2) The system's user interface for synoptic reporting was evaluated, all possible text strings were identified, and bindings of controlled text strings and SNOMED CT was created; 3) A python-based program was used to convert the PDF reports to plain text and extract synoptic text strings; 4) Free-text data were bound with SNOMED CT; 5) SNOMED CT encoded cancer data were integrated into the CDW. This project demonstrated a tractable method to extract and transform PDF-based pathology cancer data from EHR to SNOMED CT encoded data to support of research and public health. Automated data conversion from text/PDF to SNOMED CT is possible when structured text is used to create initial reports. Description It is still common for pathology reports to be stored and distributed as Portable Document Format (PDF). While this format helps user easily read the information, it causes a barrier to computation, secondary use, or electronic data exchange. That is an important issue because pathology reports contain crucial information for patient care and treatment planning. To address this, we initiated an ongoing project composed of three parts: (1) data extraction, (2) data encoding using SNOMED CT, and (3) integration into the Clinical Data Warehouse (CDW). This abstract will present findings from the process based on five cancer types: ampulla of Vater, pancreas, esophagus, lung, and prostate. Scope SNOMED CT was selected as it is currently the sole international medical terminology standard capable of representing pathology cancer data as found in pathology reports. In addition, SNOMED CT is a terminology supported for use in national and international research oriented Clinical Data Warehouses (CDW) including OMOP and PCORI. By performing this study, a practical, tractable process to extract uncoded pathology data from the electronic health record and convert to encoded, structured data for use in CDW has been demonstrated for widespread use. How SNOMED CT will be used In the encoding process SNOMED CT was used to bind (map) the extracted data from Portable Document Format (PDF). Specifically, terminology binding between the extracted data and SNOMED CT was conducted in two parts: (1) batch binding based on a predefined master file based on the synoptic report format, and (2) manual binding for free-text elements. The project follows the pathology cancer date methods proposed by the Cancer Synoptic Reporting Working Group (CSRWG) of SNOMED International and described in the Cancer Synoptic Reporting Implementation Guide. In December 2009, HIC began the initiative to become a member of IHTSDO. Consistent involvement with IHTSDO activities allowed HIC to learn more about IHTSDO as an organization, SNOMED CT, and the possible benefits to Malaysian healthcare as a whole. Over time, the use of SNOMED CT has also piqued the interest of clinicians and health managers in Malaysia. In October 2012, the Malaysian team, comprised of HIC staff representing MoH Malaysia and staff from MIMOS, a government-funded R&D ICT agency, attended the annual IHTSDO Conference and SNOMED CT Implementation Showcase in Sweden. At the event, we gained significant knowledge about how SNOMED CT was developed and continues to be further expanded, about its current application to ICT environments and about the implementation experiences of other countries. In December 2012, Malaysia formally became the 22nd member of IHTSDO, now SNOMED International. HIC bears the responsibility to produce and disseminate trustworthy health and health-related information in a timely manner on behalf of MoH to national and international agencies. To fulfill this role, HIC has four main functions: 1. Data management: HIC develops and manages the national Health Information Management System (HIMS), which collects health data for health statistical reports. It serves as a platform that allows MoH stakeholders to make evidence-based decision making and planning. HIC is also in charge of identifying new health indicators, health information and data for the MoH. Data quality is vigorously checked by monitoring the submitted reports, followed by data cleaning for granular data. 2. Publication and dissemination: HIC analyzes data collected from various HIMS and produces reports for the MoH. It also manages requests for health information from national and international agencies. 3. Health informatics standards: HIC ensures integration and interoperability between various health facilities, especially in collecting information from primary sources for secondary use. HIC manages development, maintains versioning, reviews requests, provides training and promotes the use of health informatics standards. 4. Strategic health information planning: HIC plans, develops and manages related projects in gathering health and health-related information for MoH, such as leading the Malaysian Health Data Warehouse (MyHDW) project and the Geographical Information System. The Ministry of Health (MoH) Malaysia immediately began planning its approach for developing and implementing SNOMED CT after becoming a Member of the organization. The decision was made to try and meet the needs of clinicians and health managers interested in using SNOMED CT for clinical and secondary data analysis. There is the belief that providing early results within the first one or two years after adopting SNOMED CT will encourage a high level of buy-in from stakeholders. New ICNP-SNOMED CT Nursing Practice Refset is first product from recent agreement Back 21 Oct 2021 Back OCTOBER 21, 2021 – SNOMED International and the International Council of Nurses (ICN) announce the production release of a new reference set, the International Classification of Nursing Practice (ICNP) SNOMED CT Nursing Practice Refset. In August 2020, SNOMED International and the International Council of Nurses (ICN) joined forces in a groundbreaking agreement for the International Classification for Nursing Practice (ICNP) to be managed, produced, released and distributed by SNOMED International. ICN retains ownership of ICNP and the definition of its content so that it meets the needs of the global nursing profession. The October 2021 production release of the ICNP SNOMED CT Nursing Practice Refset is the inaugural release following the 2020 agreement. ICNP, which has been in use for more than 30 years, is a standardized terminology that enables nurses to describe and report their practice, including diagnoses, interventions and outcomes, in a systematic way. The resulting information is used to support care and effective decision-making, and to inform nursing education and health policy. ICNP content has been incorporated into SNOMED CT and is being released as a reference set by SNOMED International on behalf of ICN. This Production release is based on the 2019 release of ICNP and the July 2021 International Edition Release of SNOMED CT. Subject matter experts from both organisations and the new ICNP Editorial Board at ICN have informed the decisions about additions and changes to SNOMED CT as a result of the incorporation of ICNP. To ensure fit with international requirements and other sources of terminology, the SNOMED International Nursing Clinical Reference Group has also provided advice and expertise. To accompany the reference set, equivalence tables reflecting ICNP content and equivalent SNOMED CT content have been created to show the correspondence between the content in the 2019 release of ICNP and the content in the SNOMED CT reference set. Provision of the equivalence tables supports the migration of ICNP data into SNOMED CT for those not using ICNP in SNOMED CT-enabled systems. This inaugural reference set release capitalises on ICN’s nursing terminology, which is defined by the nursing profession, and SNOMED International’s global clinical validation processes, licensing platform, quality assurance approach and established distribution tools. It has been made available for reference. On March 11, 2020, the World Health Organization formally characterized Coronavirus, COVID-19, as a global pandemic and health systems globally are continuing their efforts to manage the outbreak. SNOMED International recently announced an interim release of the SNOMED CT International Edition outside of its customary release cycle to include updated COVID-19 content as a means of lending its support to the ongoing global effort. Without reservation, SNOMED International will also include this COVID-19 content, in its recently released Global Patient Set (GPS), a managed list of existing SNOMED CT unique identifiers, fully specified names (FSN), preferred terms in international English, and status flags, made available at no cost to users. Please note, this content will appear as part of the GPS in the next release in September 2020, but is now available and can be used under the same open license, (the Creative Commons Attribution 4.0 International License). Following the announcement from WHO regarding the Emergency use of ICD-10 codes for the COVID-19 disease outbreak, the map record is also included in the March 2020 interim SNOMED CT International Edition release. SNOMED International and international health research network OHDSI collaborate Back 7 Jun 2022 Back The Observational Health Data Sciences and Informatics (OHDSI) community and SNOMED International have formalized their long-time relationship with a five-year collaborative agreement that will benefit both of their user communities. OHDSI is a non-profit, global research collaborative that provides open-source solutions and products to enable analysis and sharing of health and observational data. Coordinated by the Columbia University Department of Biomedical Informatics, its mission is to improve health by empowering a community to collaboratively generate the evidence that promotes better health decisions and better care. SNOMED CT is a core terminology within OHDSI's common data model: Observational Medical Outcomes Partnership (OMOP), allowing the use of other terminologies and classifications through computable linkages. Effective April 29, 2022, the collaboration provides OHDSI and its user community with comprehensive ontologies on specific healthcare domains and content such as devices, social determinants of health, disease severity scores and modifiers of cancers, as well as better concept definitions and resolutions of composite concepts in large-scale observational research. In return, OHDSI and its user community can provide SNOMED International with information and feedback on clinical validation, frequency of use data, and validation of SNOMED CT content modeling. Ultimately, continuous feedback shared regarding identified content gaps will benefit both user communities as we move forward together. The collaboration supports SNOMED International’s Member and stakeholder-driven five-year strategy, which includes genomic content collaboration as well as engagement with the research community. The work of OHDSI looks to showcase the use of SNOMED CT for the purposes of data analytics, supporting both healthcare research and audit, with the aim of enhancing healthcare globally. “Both OHDSI and SNOMED International are working towards creating a healthcare environment that provides both patients and clinicians the real-world evidence needed to make informed and important decisions,” said George Hripcsak, Vivian Beaumont Allen Professor and Chair of Columbia University’s Department of Biomedical Informatics. “SNOMED plays a critical role by delivering the comprehensive health terminologies necessary for OHDSI to generate reliable and reproducible evidence. Our community is thrilled to formalize this partnership so we can continue this important work.” SNOMED International CEO Don Sweete describes the collaboration with OHDSI as a major step forward supporting the global adoption of health data standards and terminologies. Supporting scientific research, networks and initiatives, SNOMED CT’s structured terminology strengthens tools used to produce research findings that drive improved patient outcomes. “Although we have worked with OHDSI extensively over the past years, formalizing our relationship offers extensive benefits to each organization’s stakeholders and we are excited about the engagement opportunities and the quality improvements the relationship will drive,” he said. “As well, our vision statements closely align: OHDSI is working toward a world in which observational research produces a comprehensive understanding of health and disease, while SNOMED International envisions a near future in which clinical terminologies will be used globally, resulting in better health and improved patient outcomes and supported by one language of health.” As part of the agreement, representatives from each organization will ensure the delivery of joint work, exploring areas where they can collaborate to highlight areas of clinical and informatics research and showcase the functionality of SNOMED International and OHDSI products. As an initial step, both parties will develop a two-year work plan that may include: use of SNOMED CT for research and analytics; evidence of benefits of SNOMED CT in use; SNOMED CT and derivative quality improvement; content collaboration; open engagement between clinical and research communities between the two organizations; and joint communications regarding the collaboration and work. SNOMED CT will be available to OHDSI and its user community throughout the term of the agreement for use in OHDSI products available on OHDSI’s GitHub repository . SNOMED International will also continue to work with OHDSI to provide SNOMED CT Development Licenses to OHDSI users in SNOMED International non-Member countries. Description Since 2019, the implementation of the Helseplattformen Electronic Health Record (EHR) has been underway, heralding a significant leap in healthcare technology within the Central Norway region. This system is built on the foundation of Epic Electronic Patient Journal (EPJ). In May 2022, Trondheim municipality was the first to use the new EHR system. Today Helseplattformen is operative for 39200 health workers, 9 hospitals, 34 municipalities, 71% of the population in Central Norway, 2 GP offices, 2 emergency care centers, all hospital labs and pharmacies.
