    SNOMED International releases an open terminology to broaden clinical data interoperability Back 30 Nov 2022 Back London, United Kingdom -- Roughly one year ago, SNOMED International declared its intention to extend the core of SNOMED CT’s structured clinical terminology to deliver an open, standalone terminology in support of the International Patient Summary (IPS) for non-SNOMED International Members. The organization has now issued the first production release of SNOMED International’s IPS Terminology that adds the hierarchy, defining relationships and extra synonyms from the SNOMED CT International Edition to form a sub-ontology of SNOMED CT that can be loaded into a terminology server for the specified scope of the IPS. The IPS itself is an extract of an electronic health record containing essential healthcare information to support cross-border and unscheduled patient care. The IPS also has the potential to be used in many other scenarios where a patient crosses health information systems, like travel between provinces/states, cities or organizations that use a different Health Information System. From a technical perspective, the IPS is a minimal, non-exhaustive set of data elements defined by ISO/EN 17269 and delivered by HL7 in both CDA and FHIR using a curated set of SNOMED CT terms. Since 2019, predating the organization’s IPS Terminology initiative, SNOMED International has made a free set of SNOMED CT codes available (the HL7 IPS free set), as part of its Global Patient Set (GPS) to support the implementation of the HL7 FHIR IPS specification . The GPS is a ‘flat list’ of SNOMED CT codes and terms without hierarchy or relationships, whereas the IPS Terminology brings all the benefits of the SNOMED CT structure. CEO Don Sweete shares his insights on how the IPS and the IPS Terminology are relevant in today’s changing global healthcare environment. “In response to the interest that governments globally have shown in implementing the IPS to enhance timely cross border health information flow and interoperability, SNOMED International has prioritized the creation and release of the IPS Terminology, openly available with content from SNOMED CT.” He continued on to state that, “We are at a point where enabling digital healthcare systems worldwide to work together seamlessly is critical. To advance these efforts, we have chosen to dedicate our resources to achieve the goal of interoperability by equipping the IPS, already one of the best examples of international collaboration among standards bodies, with the full capabilities that SNOMED CT offers. We will continue our collaborative efforts in this space as the needs of global health care systems continue to evolve.” Throughout 2022, SNOMED International has been configuring its approach to making the IPS Terminology a tangible artifact in its lineup of products and services. First released in its beta version in April 2022 for focused stakeholder testing and feedback, the IPS Terminology is accompanied by a User Guide to support implementers in non- Member regions to understand the features, capabilities and limitations of the IPS Terminology package. Providing an overview of the IPS Terminology's scope and features, the Guide also provides IPS Terminology use cases as well as direction on how to implement the IPS Terminology in a terminology server, and how to upgrade to a full SNOMED CT edition if additional terminology content is required. It is worth noting that current SNOMED CT users, those located in a SNOMED International Member region or those with an Affiliate license to a complete SNOMED CT edition, should avoid use of the IPS Terminology and instead, implement their IPS solutions using a full edition of SNOMED CT. This reference set identifies the subset of key concepts from the IPS Terminology and is available as a supplementary package from SNOMED International's MLDS service . Finland’s commitment to SNOMED CT sets a foundation for clinical data to be exchanged accurately and consistently across Finnish health and social care settings, further extending the reach of structured clinical terminology throughout the European region. To make best use of SNOMED CT with available resources, Finland has chosen not to translate the whole terminology. Instead, the aim is to focus on two national reference sets that meet local needs in areas where there is international experience and models showing implementation success. The first target is an obvious one: standardizing the terminology and codes for anatomic pathology in the clinical laboratories. Finnish pathology laboratories have used SNOMED 2 since 1980’s. But lack of coordination eventually resulted in several dialects and variance between hospitals. Now the ongoing project aims at national reference set to help the biobanks, regional and national quality registers and structured reporting on individual patient basis. The other chosen application for SNOMED CT is patient problem list. Finland decided to use CORE Problem List as foundation for the local effort. CORE Problem List originated from a respectable group of medical institutions in the U.S. and is maintained by the National Medical Library thus making the acceptance of the reference set by clinicians so much easier. The ongoing project translates terminology into Finnish and when finished will cover terms for both professionals and patients. Finland's aim is to have these first two applications ready at the first half of 2020. SNOMED CT National Release Center will gradually take off during 2019 and 2020 to meet the needs for services and support during SNOMED CT local implementations.     TermMed IT Back TermMed IT Vendor Overview Our involvement in healthcare informatics started in the early ’90s, focused on patient care, medical education and clinical investigation. Soon we got aware about the challenges of information exchange between clinical information systems, and became involved in the development of interoperability standards with a focus on their application in Spanish-speaking environments. In 1995 we agreed with the College of American Pathologists (CAP) to start translating SNOMED III into Spanish as a volunteer effort. This collaboration evolved with the translation of SNOMED RT and SNOMED CT, resulting in the publication of SNOMED CT Spanish Edition in 2004. We continued to maintain that edition on behalf of CAP until the IHTSDO obtained the rights to SNOMED CT International Edition (which included the Spanish version). Now, we still collaborate in the maintenance of the Spanish translation as contractors to the IHTSDO, although in the last ten years the focus of our work has been the support of the content and technical processes involved in SNOMED CT International Edition maintenance. Additional information The Member led Drug Extension User Support Group (DEUSG) of SNOMED International identified the need for a map between EDQM Standard Terms and SNOMED CT dose forms to serve the following use cases: To represent real / branded medication products in a SNOMED CT drug extension, based on the registered product information (where EDQM Standard Terms dose forms are used for regulatory purposes). To map medicinal products from a national or local medicinal product dictionary to internationally agreed SNOMED CT Clinical Drugs, for interoperability purposes, such as the sharing of medication lists for cross-border patient-care, pharmacovigilance and clinical research. To map from an IDMP-compliant regulatory database of medicinal products to SNOMED CT drug concepts (which include the dose form). The work has been completed on aligning the SNOMED CT dose forms with those in EDQM Standard Terms, where in scope of SNOMED CT.     SNOMED International and LOINC® announce renewed collaboration agreement and upcoming production release of the LOINC Ontology Back 11 Mar 2025 Back SNOMED International and LOINC® by Regenstrief announce their renewed agreement and upcoming production release of the LOINC ® Ontology: A LOINC and SNOMED CT interoperability solution . Renewed commitment Although the main focus of the renewed collaboration agreement is the release of the LOINC Ontology, it also continues the work initiated in 2022, including the adoption of working practices that facilitate the use and interoperability of the terminology standards both organizations produce. It will also focus on ensuring that stakeholders/users globally have clear direction on the use of the standards, whether together or individually, and that an effective interoperability solution is available to users when both standards are implemented in the same system. The upcoming release is the result of the renewed collaboration between the two organizations that began in 2022 and continued in 2023 with the technical preview of the ontology , building on the relationship formed more than a decade ago. The ontology, colloquially referred to as the LOINC Extension, is slated for release at the end of March 2025 and will initially cover quantitative laboratory tests. After the initial production release, Regenstrief plans to issue the release twice a year (March and September) following the twice annual LOINC release . The LOINC Ontology supports providers and users who implement different combinations of SNOMED CT and LOINC in health information systems and allows them to meet clinical and regulatory requirements in a single solution. An extension of SNOMED CT’s ontological framework, the ontology creates SNOMED CT and LOINC codes for all concepts that are shared between the terminologies. SNOMED CT provides the computable framework and LOINC provides laboratory and pathology content in an understood format to countries that do not currently use LOINC. The goals of the collaboration are to improve health and better care through standardized terminology and to make it easy for implementers to take a unified approach to implementing both standards. It minimizes duplication of effort between Regenstrief and SNOMED International, gives implementers a choice of which codes to use, and provides a single solution that meets clinical and regulatory requirements globally. As part of this joint effort, SNOMED International subject matter experts have worked with the LOINC team to provide ontological guidance, modelling, relevant content specialization and technical support. Regenstrief, however, continues to own and manage the ontology. The March 2025 release will include more than 35,000 concepts, including quantitative lab tests and the top 200 most used lab tests ordered. Domains of information (lab, clinical, documents, radiology, survey instruments) will be added incrementally. We will link existing SNOMED CT concepts to existing LOINC content where applicable and new LOINC codes will be created as needed. LOINC Parts will be mapped to SNOMED concepts, and new SNOMED CT concepts will be created as needed. Over the next few months, both organizations will also focus on developing implementation resources, including jointly developed guidance, tangible pilots and opportunities for community engagement, to better understand implementers’ experiences. LOINC , short for Logical Observation Identifiers Names and Codes, is a worldwide standard used in 193 countries and available in 20 languages for identifying health measurements, observations and documents enabling the identification, exchange and collection of data across health systems. Created and maintained at Regenstrief Institute , the system is free to use. Sharing a common goal of developing and provisioning standards for the exchange of interoperable clinical and related health information, HL7 International and SNOMED International have agreed to target their collaboration to further support the implementation of SNOMED CT in HL7 artefacts, consistent with our mutual commitment to advance global health data interoperability. Per the recently signed agreement, both organizations have extended their commitments to develop projects satisfying the use of SNOMED CT with HL7 Fast Healthcare Interoperability Resources, or FHIR®, and other HL7 products. In particular, the two organizations will continue to work together to maintain the HL7 IPS Free Set in line with new requirements and changes to SNOMED CT. As the need for collaboration among global health standards is highlighted, most recently illustrated in the G7 digital health communique , there is more of a need than ever before to continue our efforts and offer solutions to global stakeholders. HL7 CEO Dr. Charles Jaffe, MD, PhD, underscored the significance of this agreement. “The Covid-19 pandemic has greatly heightened the demand for more seamless health data exchange. Our collaboration with SNOMED International is critical to fostering data interoperability. Our long-standing collaboration with SNOMED and the resulting incorporation of SNOMED CT vocabulary within HL7 standards, including FHIR, will advance us to a world in which everyone can access the right data where and when they need it." SNOMED International CEO Don Sweete reflects on the long-standing relationship that both organizations have contributed to and nurtured over the past seven years. “The joint commitment and work produced by our two organizations offers tangible interoperability tools that meet the breadth of content and messaging needs that we are observing globally“ he said. “Collaboration agreements of this nature are an important step in answering the call to action being expressed recently by G7 leaders to address pressing interoperability issues.” Under the agreement, target activities have been planned up to July 2023, at which point a review of delivered work products and upcoming activities will be undertaken. The focus of this first stage of work includes licensing arrangements, change requests and terminology binding for the use of SNOMED CT in HL7 products, maintenance and updates to the HL7 IPS Free Set, and promoting the use of SNOMED CT and FHIR in international use cases. Additional activities will support technical collaborative efforts at HL7 FHIR Connectathon events and prospective educational opportunities put forward by either organization. Extending across the length of the agreement is a joint openness to actively identify further joint working opportunities.
