49 reference sets have been developed and include: health concern, reason for visit, intervention and referral.

Scope

SNOMED CT was augmented by Canadian specific content in the development of content to support most of the reference sets, enabling a consistent way of capturing, sharing, and aggregating health care data across specialties and sites of care.

How SNOMED CT will be used

SNOMED CT is the most comprehensive, multilingual health care terminology in the world. It has the ability to grow, change and adapt to meet the needs of the clinician. The SNOMED CT model allows a clinician to state almost anything about a patient in a way that is unambiguous, reproducible and useful both inside and outside the care setting. SNOMED CT provides a robust terminology model to facilitate aggregation and querying of data for health system use.
