Embedding SNOMED CT into the critical care workflows of a multi-hospital NHS Trust has enabled structured, high-quality data capture that directly supports clinical decision-making, patient safety, and service improvement. Critical care patients often present with multi-organ dysfunction, rapidly evolving conditions, and complex comorbidities, generating vast volumes of clinical data. This includes continuous physiological monitoring, laboratory results, imaging, medication administration records, and multidisciplinary documentation. In this high-acuity, high-data environment, the routine recording of structured data - including diagnoses, problems, and procedures - using SNOMED CT within Oracle Health Millennium provides clarity in understanding patient journeys and ensures that key clinical information is accessible, interpretable, and interoperable. Structured SNOMED CT data supports care bundle tracking, longitudinal patient profiling, and feeds into national registries such as ICNARC, enabling benchmarking, audit, and informatics research. This approach accelerates the creation of data, information, and evidence to improve outcomes and inform future care.

Description

This implementation spans the critical care network encompassing multiple hospital sites within a large tertiary, university hospital trust, . The project aimed to standardise clinical documentation, explore diagnostic patient cohorts, provide near-realtime data insights and support further analytics. By embedding SNOMED CT into Oracle Health Millennium workflows, the Trust created a foundation for consistent, high-quality data capture that supports both frontline care and secondary uses such as audit, research, and service planning.

Scope

SNOMED CT was selected for its breadth of clinical coverage, robust description logic, and ability to support semantic interoperability across systems. Its integration with Oracle Health Millennium enabled structured data capture at the point of care, supporting both clinical decision-making and downstream analytics. SNOMED CT's alignment with national standards and its adaptability to local pathways made it the ideal terminology for this high-impact, multi-site deployment.

How SNOMED CT will be used

SNOMED CT concepts were used to structure documentation of diagnoses, problems, and procedures, within hospital wide EHR Oracle Health Millennium. These coded entries support direct care tracking, longitudinal patient profiling, and structured data was leveraged for retrospective analysis, benchmarking, and quality improvement. The use of SNOMED CT ensures semantic consistency across sites and disciplines, enabling scalable informatics innovation.
