Description

To capture diagnosis in problem list using a robust standardized clinical terminology that is mapped to ICD10, and ICD10 code is used for eRx submission while SNOMED CT terminology populates the patient problem list in the EMR.

Scope

Capturing diagnosis in patients EMR problem list (using SNOMED CT terminology), which in turn feeds ICD10 code for eRx submission.

This involves multiple speciality in the Outpatient Department which includes:

  • Primary Care

  • Pediatrics

  • Nephrology

  • Dermatology 

  • Internal Medicine (Rheumatology, Infectious Disease, Geriatrics, Pulmonology, Gastroenterology)

  • General Surgery

  • Neuroscience

  • OBG

  • Heart Center

  • Diabetes Center

  • Orthopedics

  • ENT

  • Ophthalmology

  • Urgent Care

How SNOMED CT will be used

ICD10 MAP is used. This initiative encourages the use of standardized terms to populate patient problem list in the EMR while also offering ICD10 codes to be submitted to eRx portal.

It is a recognized standardized clinical terminology. It offers robust clinical terms when capturing data in patient medical chart and supports interoperability.
