We are working on a rare disease portal. In this portal patient apply for medical assistance.
Scope
In the future, the application mapped with other system, will map directly with snomed-ct id's. If other organisations need data, then data is given according to snomed ct id's.
How SNOMED CT will be used
All patient diseases are mapped using snomed ct id's. All medical details are also mapped with snomed-ct id's.
Why SNOMED CT will be used
Because it gives all diseases and related medical terms IDs like (weight,height and many more).
