- SNOMED CT Clinical Implementation Guide for Cancer Synoptic Reporting supports accurate documentation and improved information-sharing
SNOMED CT Clinical Implementation Guide for Cancer Synoptic Reporting supports accurate documentation and improved information-sharing Back 9 Oct 2024 Back SNOMED International is pleased to announce the release of the SNOMED CT Clinical Implementation Guide for Cancer Synoptic Reporting , a comprehensive resource to facilitate standardized and structured reporting in cancer care. Designed to enable healthcare organizations to improve the quality of information captured, facilitate data exchange, support research initiatives, and enhance collaboration among healthcare providers, the guide provides clinicians with an overview of the content of SNOMED CT that supports cancer synoptic reporting and highlights the role of SNOMED CT in standardizing and enhancing cancer reporting practices. Guide scope and intended use The scope of the guide is specific to the creation of SNOMED CT concepts necessary to unambiguously represent the data elements required for cancer reporting for all adult and pediatric solid tumors as published by the College of American Pathologists (CAP) and the International Collaboration on Cancer Reporting (ICCR). The content is specific to structure pathology reporting of malignant neoplasms as specified by the CAP and ICCR. It provides general use cases, including clinical application examples and scenarios; information models and terminology bindings; and considerations related to the technical application of cancer synoptic reporting forms with SNOMED CT. It also describes three approaches for cancer synoptic data recording – paper-based forms, distributed electronic forms and a centralized reporting platform. The International Collaboration of Cancer Reporting (ICCR), formed in 2011, has strong support from a number of major cancer organizations globally and has grown rapidly. Notably, its membership includes the Union for International Cancer Control and the International Agency for Research on Cancer. SNOMED CT is a comprehensive, multilingual healthcare terminology created for use by healthcare professionals to capture the care of individuals in an electronic health record and facilitate sharing, decision support and analytics, to support safe and effective health information exchange. SNOMED CT Clinical implementation guides are designed to assist both clinical and technical users in using SNOMED CT for specific clinical purposes. Synoptic reporting According to the American College of Surgeons , synoptic reports “present information as discrete data elements and associated responses. The synoptic report starts as a blank template, which is completed by the user. Each data element acts as a question that requires an answer selected from a list of predetermined responses. Once the template has been completed, each entry can be separately identified and extracted as a distinct unit of information.” Cancer synoptic worksheets, checklists and tissue pathways are the prevailing form of text-based structured cancer reports based on anatomic pathology (histopathology) and molecular (genomic and proteomic) pathology assessments. The Cancer Synoptic Reporting Clinical Implementation Guide follows the establishment of the Cancer Synoptic Reporting Project Group , which was formed in 2020 to develop comprehensive SNOMED CT concepts suitable for structured pathology reports. The group includes pathology experts who provide input from the community of practice on the development, maintenance and use of SNOMED CT in this specific domain. It is thanks to the hard work of this group that this Clinical Implementation Guide is now being published. They will update it over time based on feedback. Helping clinicians deliver better care SNOMED International CEO Don Sweete calls the guide’s release an important step in supporting clinicians who provide cancer care. And while ensuring that critical data elements are accurately documented and easily shared across different healthcare settings is one element of enabling better care, “standardized terminology and structured templates also help healthcare professionals streamline the documentation process, reduce variability in reporting practices, and promote adherence to evidence-based guidelines,” he notes. Technical considerations In addition to content relevant to clinicians, the guide also provides detailed descriptions for vendors and system developers wishing to implement the approach to cancer reporting within systems. Other features of the guide include: Examples of how synoptic reporting can benefit specific scenarios such as hematologic malignancy evaluations, cervical cancer screenings and neuro-oncology tumor resections, enabling synoptic reports to play a crucial role in guiding clinical decision-making and optimizing patient outcomes; A demonstration of how existing standards such as SNOMED CT and HL7 FHIR Questionnaires can be effectively employed to create implementable and shareable representations of synoptic reporting forms; and A roadmap for healthcare organizations looking to implement SNOMED CT in cancer synoptic reporting. SNOMED International is an international not-for-profit organization that develops and promotes the use of SNOMED CT, a comprehensive, multilingual healthcare terminology created for use by healthcare professionals to capture the care of individuals in an electronic health record and facilitate sharing, decision support and analytics, to support safe and effective health information exchange. - One step closer to interoperability: Applying SNOMED CT’s engine to the IPS
One step closer to interoperability: Applying SNOMED CT’s engine to the IPS Back 25 Nov 2021 Back Since the date of issue, this information is now out of date and has been archived. It has been made available for reference. At SNOMED International’s recent October Business Meetings held in London, the organization’s governance bodies enacted a decision to extend the core of SNOMED CT’s structured clinical terminology to deliver an open, standalone sub-ontology to support the scope of content within the International Patient Summary (IPS.) The IPS is an electronic health record extract containing essential healthcare information for use in the unscheduled, cross-border care scenario, as well as for local, regional and other care scenarios. It is a minimal, non-exhaustive set of data elements defined by ISO/EN 17269 and delivered by HL7 in both CDA and FHIR using a curated set of SNOMED CT terms. There is a groundswell of support across all health sectors to increase the portability and usability of patient information for the purpose of safe health care delivery. In 2019, SNOMED International and HL7 International announced the formalization of a license agreement in which a relevant ‘Free for Use’ Set of SNOMED CT coded concepts would be used within the HL7 IPS. Most recently, we watched as G7 leaders collaborated to release a communique on the dire need to progress a global health interoperability agenda. IBM offers a working definition of interoperability as “the timely and secure access, integration and use of electronic health data so that it can be used to optimize health outcomes for individuals and populations.” The G7 communique, which highlighted the importance of enabling digital healthcare systems worldwide to work together seamlessly as patients move between providers, facilities and even countries, is an impactful statement that rippled throughout the global health community. A charge taken up by the Global Digital Health Partnership, it is one SNOMED International is eager to support. Embracing a collaborative approach, “SNOMED International has been pleased to continue to work with HL7 International and partners across Europe and beyond to define SNOMED CT content for use in the International Patient Summary,” offered SNOMED International Management Board Chair, Joanne Burns. Continuing to act in the spirit of the IPS Free Set , SNOMED International has committed to create and release an openly available IPS sub-ontology in the first half of 2022 to enhance the existing cross border movement of information, and ultimately health system interoperability. Unlike SNOMED International’s Global Patient Set , a flat list of SNOMED CT codes and terms, an IPS sub-ontology will provide implementers with a product that can be used in healthcare solutions using the power of SNOMED CT through its query language and hierarchies for the specified scope. Use of the IPS sub-ontology will allow for more effective use of clinical data analytics and decision support, and for Artificial Intelligence applications. Alex Elias, Chair of SNOMED International’s General Assembly, the organization’s Member governance body, has observed a significant increase in discussion regarding the IPS. “2021 has seen increased interest by governments and Health and Care organizations globally for implementing the IPS to enhance timely cross border health information flow and interoperability. This has been a primary driver in SNOMED International supporting this recent initiative to make the IPS sub-ontology openly available with SNOMED CT content.“ An organization with an extensive history and active program of collaboration, SNOMED International CEO, Don Sweete, has played a pivotal role in positioning the IPS sub-ontology as a ‘soon to be achieved’ reality. “As the G7 Health Ministers recently indicated, the importance of enabling digital healthcare systems worldwide to work together seamlessly so patients don’t suffer as they move between providers, facilities and even countries is a sentiment that has rippled throughout the global health community”, offered Sweete. He went on to state that, “continued work with fellow health standards development, national, clinical and technical entities, SNOMED International will dedicate resources to achieve the goal of digital health interoperability.” Sweete added, “equipping the IPS, already one of the best examples of international collaboration among standards bodies, with the full capability of SNOMED CT’s ontological design is a significant action that we can contribute to achieving health information access gains for patients.” Over the coming months, SNOMED International is formalizing the steps and due diligence required to make the IPS sub-ontology available for broad release. Throughout this period, SNOMED International will continue to define the IPS sub-ontology, from content through to its release and maintenance approach for launch in the first half of 2022. - ASSESS CT declares SNOMED CT is the best available core reference terminology in Europe
ASSESS CT declares SNOMED CT is the best available core reference terminology in Europe Back 9 Jun 2017 Back The goal of the ASSESS CT report, released in early 2017, was to make a significant contribution to the debate on eHealth semantic interoperability in Europe. The substantial analysis and research invested by ASSESS CT has identified SNOMED CT the core reference terminology for EU-wide deployment; a significant piece of the puzzle needed to accomplish eHealth interoperability. SNOMED International recognizes ASSESS CT for its leadership throughout this initiative and welcomes further collaboration opportunities to support achievement of semantic interoperability across Europe, and globally. Please see the SNOMED International Response linked below for more information. The 5 recommendations include: Any decision about the adoption and role of terminological resources, including SNOMED CT, must be part of a wider, coherent and priority driven strategy for optimising the benefits of semantic interoperability. in health data, and of the overarching eHealth strategy of the European Union and its Member States. SNOMED CT is the best available core reference terminology for crossborder, national and regional eHealth deployments in Europe. SNOMED CT should be part of an ecosystem of terminologies, including international aggregation terminologies (e.g., the WHO Family of Classifications), and user interface terminologies, which address multilingualism in Europe and clinical communication with multidisciplinary professional language and lay language. The adoption of SNOMED CT should be realised incrementally rather than all at once, by developing terminology subsets that address the interoperability requirements for priority use cases and expanding these sets over a number of years. Mechanisms should be established to facilitate and coordinate European Member State cooperation on terminology and semantic interoperability, including common areas of governance across national terminology centres, eHealth competence centres (or equivalent national bodies).
