SNOMED CT is used to code drugs and pathology findings in the Catalan region in Hospital Clínic of Barcelona.
Scope
SNOMED CT will contribute to semantic interoperability and is used as a code system to store clinical information (i.e. storing SNOMED CT identifiers in your patient records), as a common terminology for communication (i.e. sending SNOMED CT codes to other healthcare providers) in areas of pharmacy, clinical documentation, decision support and interoperability.
Use cases are medication list, clinical documentation, Order Communication and Result Reporting, Medication/ Allergy Management. To reach implementation a subset of SNOMED CT is being used and a catalan pathology subset has been created.
