However, adherence It has always been a challenge, especially among elderly patients. It is essential to consider the patient, the medications, the providers of health services and socioeconomic factors, to find adequate and individualized interventions to overcome these problems. Therefore, it is necessary to ensure adherence to prescribed medications so that the patient can receive the maximum therapeutic benefits. For successful prescriptions, health professionals and researchers must use methods within their practical limits to improve adherence to treatment, also can address to professionals to choose the most appropriate decisions. There is a need to develop terminological standards of adherence, for interoperability and reusability in comprehensive care settings and therefore, analyze these data later and generate patterns of behavior to prevent problems in such adherence. Reduce poor adherence can have a meaningful impact on the clinical outcome, which contributes to the substantial worsening of the disease, the increase in health care costs, and even death. With an increasing number of medications, poor adherence is a growing concern, severely harming the benefits of current medical care. On the other hand, the increasing number of drugs prescribed at hospital discharge is correlated with non-adherence, and a high percentage of patients do not understand the purpose of their medications. Provide instructions clear and simplify the medication regimen as much as possible could be a simple way to facilitate the patient's follow-up of a medication regimen, and the intervention could be evaluated in randomized clinical trials.

Description

The objective of this proposal is the development of an ontology of domain of terminology of adherence standard-based, of all the concepts related to adherence to pharmacological treatments, in the context of the integrated healthcare plans.

Scope

The aims are that all the adherence concepts managed are represented in Snomed CT if they are specific to the biomedical and health field. Snomed CT will be extended to adherence data sets so that the information related to the adherence can be processed and semantically connected with the relevant information on what the clinical decisions of the plans are implemented, based on clinical guidelines and other biomedical knowledge sources.

Snomed CT is the recognized universal terminology and incorporates in its scope the appropriate clinical, behavioral, social, and pharmacological concepts to respond to the semantic interoperability needs of this domain.
